ACORD 25 Form

ACORD 25 is the standardized 'Certificate of Liability Insurance' form used across the U.S. insurance industry. Because nearly all COIs use it, its layout is predictable — insurer, insured, coverage grid, limits, and the certificate holder box.

Knowing where each field sits on the ACORD 25 is what makes automated COI reading reliable: coverage limits, policy dates, and endorsement checkboxes are always in the same regions of the form.
